REFERRALS

Under the guidance of the Shared Care Committee (a collaborative partnership of the BC Ministry of Health and Doctors of BC), a working group of cardiologists, family physicians, patients, clinic assistants, and administrators have developed a regional referral form to improve patient care.

This new form streamlines and replaces several dozen other forms and allows specialists, emergency doctors, family physicians, and nurse practitioners the ability to refer directly to both private offices as well as to any of the 30 ambulatory cardiology clinics located throughout Vancouver Coastal Health and Providence Health. Concise clinic descriptions and an acknowledgment of referral component are also included, serving as an education piece to the referring provider.
